Joncris Sentinel Services, in conjunction with Foxleigh Mining, have developed a Electronic Risk Assessment Database that fulfills the requirements of the Recognised Standard 02, Control of Risk Management Practices and based on the Minerals Industry  Risk Management Guidelines.

The front page allows you to see what risk assessments have been conducted. Great for document control and enabling rapid reviews of previous risk assessments.

The first page ensures the scope of the risk assessment is clearly defined, an issue that we see time and time again that needs improvement.

This page captures the job steps and also forces the team to clearly define the hazard in a Failure Modes and Criticality Analysis. Once hazards are entered, the next step is to enter Controls.

This page is for entering Existing Controls. The risk score is calculated with consideration of Consequence, Probability and Exposure. The team is also forced to evaluate the Effectiveness of the Controls.

If the risk is Unacceptable, then Additional Controls need to be included on this page. Again the Risk Score is calculated and the Control Effectiveness is evaluated. An accountable person is nominated for the control implementation.
